About this role
Registered Nurse (RN) Care Manager – Lakewood Family Medicine. Full-Time | 0.8 FTE or 64 hours/pp | Exempt/Salaried. 8 x 8-hour Day shifts/pp, Flexible Start Times. UP Health System - Marquette is part of Lifepoint Health. As a Care Manager RN, you will identify targeted populations and implement clinical interventions, collaborate with PCP and healthcare team, provide self-management support, facilitate monitoring of high-risk patients, and coordinate care between PCP and acute care hospitals, specialists and post-acute care services.

- RN
- Current Michigan RN license
- Associate's degree from an accredited nursing school (bachelor's preferred)
- Basic Life Support Health Care Provider (BLS-HCP) certification
- 2 years experience in adult medicine and pediatric patients
