About this role
This is a part-time (approx. 10 hour/week) RN position with a lot of flexibility.
Support and Services at Home (SASH) is a program of the Vermont Blueprint for Health initiative to reform the delivery of health care in our state. SASH is embedded in affordable-housing sites throughout Vermont and serves primarily Medicare-eligible residents. Participation in SASH is voluntary, with programs and services provided free of charge.
The SASH program coordinates the resources of social service agencies and health providers to help seniors live more independently. Participants in Valley Village's SASH program live at Valley Village Senior Housing in Townshend, Putney Meadows, or in their own homes throughout the West River Valley.
The SASH Wellness Nurse oversees wellness care and coaching for SASH participants so they can successfully self-manage their care and remain living at home. Key responsibilities include:

- Conducting comprehensive health and wellness assessments of participants and assisting in the creation of individual healthy living plans and the community healthy living plan.
- Working in coordination with other SASH team members to provide support and coordinated care for participants returning home after a stay in a hospital or rehabilitation facility;
- Providing health monitoring and preventive services such as blood-pressure readings, heart- and respiratory-rate readings, weight monitoring and referrals as needed.
- Providing wellness education and supportive counseling regarding chronic disease and medication self-management, long-term-care planning and other health-related matters.
- Working closely with the SASH Coordinator to address participants’ needs and communicating with the SASH team, family members and physicians as appropriate.
