*Job Overview* We are seeking a dedicated and compassionate Family Nurse Practitioner or Physician Assistant to join our healthcare team. - Conduct thorough physical examinations and health assessments for patients.
- Develop and implement individualized care plans based on patient needs.
- Perform diagnostic evaluations and interpret results to guide treatment decisions.
- Provide education and counseling to patients regarding health management and disease prevention.
- Document patient encounters accurately in electronic health records (EHR) system.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Maintain compliance with HIPAA regulations and ensure the confidentiality of patient information.
*Qualifications*
- Current certification as a Family Nurse Practitioner (FNP) or Physician Assistant (PA) with an active California state license.
- Experience in primary care or family medicine preferred.
- Proficiency in EMR/EHR systems such as eCw.
- Strong clinical skills in areas including but not limited to:
- - Chronic care - Pediatrics - Geriatrics - Triage and patient assessment
- Familiarity with medical terminology, CPT coding, ICD coding (ICD-9 & ICD-10), and medical documentation practices.
- Knowledge of chronic disease management including infectious disease care and senior care practices is a plus.
- Excellent communication skills with a focus on patient education and health coaching.
