*Job Overview* We are seeking a dedicated and compassionate Family Nurse Practitioner or Physician Assistant to join our healthcare team. The ideal candidate will provide comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and managing acute and chronic illnesses. This role requires a strong foundation in clinical skills, patient assessment, and the ability to work collaboratively within a multidisciplinary team.

  • Conduct thorough physical examinations and health assessments for patients.
  • Develop and implement individualized care plans based on patient needs.
  • Perform diagnostic evaluations and interpret results to guide treatment decisions.
  • Provide education and counseling to patients regarding health management and disease prevention.
  • Document patient encounters accurately in electronic health records (EHR) system.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Maintain compliance with HIPAA regulations and ensure the confidentiality of patient information.

*Qualifications*

  • Current certification as a Family Nurse Practitioner (FNP) or Physician Assistant (PA) with an active California state license.
  • Experience in primary care or family medicine preferred.
  • Proficiency in EMR/EHR systems such as eCw.
  • Strong clinical skills in areas including but not limited to:
  • - Chronic care - Pediatrics - Geriatrics - Triage and patient assessment
  • Familiarity with medical terminology, CPT coding, ICD coding (ICD-9 & ICD-10), and medical documentation practices.
  • Knowledge of chronic disease management including infectious disease care and senior care practices is a plus.
  • Excellent communication skills with a focus on patient education and health coaching.
