RN Visit Nurse – Home Health | St. Paul Area | $4,000 Sign-On Bonus. Full-Time with On-Call and Weekend Rotation.
Compensation:
$75,000 to $100,000 per year plus a $4,000 Sign-On Bonus. Provide skilled RN home visits throughout the St. Paul territory. Complete comprehensive patient assessments. Perform wound care, medication management, injections, and other skilled nursing procedures. Develop and update individualized care plans. Educate patients and caregivers on medications and disease management. Communicate with physicians and interdisciplinary care teams. Complete timely documentation using Homecare Homebase. Monitor patient progress and coordinate ongoing care.

Requirements
- Active Minnesota Registered Nurse (RN) license
- Experience in home health, acute care, rehabilitation, long-term care, or skilled nursing
- Strong clinical assessment and decision-making skills
- Excellent communication and patient education abilities
- Self-motivated with strong organizational skills
- Reliable transportation for local travel throughout the St. Paul service area
- Mid-level experience
- Homecare Homebase experience
