About the Role
Heart and Soul Healthcare is hiring a Licensed Practical Nurse for a 1:1, in-home, private-duty assignment with an adult male client in his mid-20s in Lilburn, GA (Gwinnett County, near Stone Mountain). The client has autism and a severe intellectual disability, is non-verbal, and lives at home with an engaged family. The schedule is built around long weekend shifts: 8 or more hours on Saturday and Sunday, starting as early as 10:00 AM, plus some hours on Friday, up to 21 hours per week.
This is a behaviorally complex case with moderate medical needs. He walks independently and has no seizures, no feeding tube, and no lifting. What he needs is a calm, steady nurse who is comfortable with self-injurious and aggressive behavior, who can follow a written behavior support plan, and who will keep up his skin care, wound care, feeding, toileting, and medication routine to the letter. You will be trained on his behavior plan in person by his behavior clinician, you will have a written RN care plan, and our Director of Nursing and nursing supervisor are a phone call away.
The Schedule
- Saturday and Sunday: 8 or more hours each, starting as early as 10:00 AM
- Friday: a shorter shift, with times set together with the family
- Up to 21 hours per week, with some flexibility on start and end times
- Ideal for a nurse who works weekdays elsewhere and wants steady weekend hours; Saturday-only or Sunday-only applicants are welcome
What You Will Do

- Apply, remove, and monitor physician-ordered protective equipment (soft arm splints, padded gloves, and a protective helmet) on a timed schedule, with skin checks at every break.
- Perform daily skin assessments and basic wound care for self-inflicted scratches and wounds; apply topical medications as ordered; report new or worsening wounds to the supervising RN.
- Assist with scheduled toileting, monitor for rectal bleeding related to a chronic rectal ulcer, apply prescribed rectal medication when indicated, and track bowel movements and intake and output.
- Feed and supervise all meals on a strict gluten-free, casein-free, nut-free diet with small, soft bites; he is a choking risk and must be fed 1:1.
- Administer scheduled and as-needed medications per the MAR and physician orders, including nebulizer treatments for asthma; observe for side effects of psychiatric medications.
- Follow his behavior support plan: proactive reinforcement, neutral responses, blocking and redirection of self-injury, crisis steps, and daily behavior data collection.
- Assist with bathing, dressing, and hygiene; keep the environment clear of small objects (pica precautions) and doors secured (elopement precautions).
- Document in our electronic record (Therap) and communicate with the family, the supervising RN, and the behavior clinician.
Who You Will Support
A young man who loves music, walks, the park, the grocery store, sensory toys, his iPad, and being included. He communicates with gestures, by taking your hand, and with a tablet. He has severe self-injury (head hitting, skin picking) and can grab, scratch, pinch, and bend fingers when frustrated, which is why he wears protective equipment and has a behavior plan. Nurses who stay calm and non-reactive do very well with him. His mother is his primary caregiver, is in the home, and knows his routine in detail.
