Registered Nurse - Adult Internal Medicine Clinic

Utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with more complex needs with assistance and supervision. Assists with the management of patient care administered by other members of the nursing team. Guided by the policies and procedures of the Ambulatory Services Administration, and in collaboration with providers and other clinical staff, the Clinical Nurse 2 is responsible for providing safe, efficient nursing care to patients in an out-patient, primary care practice. Triages patients and responds appropriately according to protocol and nursing assessment. The Clinical Nurse 2 utilizes the nursing process (Assessment, Planning, Implementation and Evaluation) to provide safe and therapeutic clinical care. The Clinical Nurse 2 also provides clinical support to providers and participates as part of the health care team to assure coordination and continuity of patient care. The Clinical Nurse 2 is responsible for triage on behalf of the practice both in person and through telephone and electronic communication. S/he participates in the development and implementation of educational programs and materials for patients and their family members. S/he delivers direct patient care to a variety of patient populations. All components of nursing care will be individualized to meet the patient's physiological and psychosocial development. The Clinical Nurse 2 will play a critical role in the patient experience including optimizing clinic flow, providing in and outreach to patients and working collaboratively with the team to create a patient centered environment. S/he participates in Performance Improvement (PI) activities. They are required to learn and apply the Patient Centered Medical Home Standards and to maintain a safe environment for patients. The Clinical Nurse 2 consistently projects a professional and courteous demeanor, helps establish and maintain the highest service standards for the practice and complies with the Medical Center House and Telephone Standards. This employee will utilize computer skills in APeX and other UCSF computerized programs. This individual supports the standards of the policies and procedures of the practice, and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration, and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice. They are required to adhere to UCSF's PRIDE values and the AIDET fundamentals of communication in all patient care and stakeholder interactions.
