About this role
Adoration Hospice is seeking a passionate, dedicated Hospice RN to join our team in Cleveland, TN. Our hospice RNs provide expert, patient-centered care at a critical time in people's lives. As a RN Case Manager you will assess/monitor physical, emotional, and psychological needs of patients, create hospice care plans, direct nursing care, provide pain and symptom management, educate and support the patient's family and caregivers, collaborate with interdisciplinary team, maintain accurate and timely documentation, and participate in on-call rotation.
