Your Impactful Role as an RN Case Manager

In your pivotal role, you will

  • Manage and Schedule Visits: Enjoy the independence of managing your patient visits, traveling within the community (East Grand Forks and surrounding areas) to provide personalized, one-on-one nursing services in patients’ homes. This role allows you to make deep connections with patients, ensuring they receive the utmost care and attention.
  • Interdisciplinary Collaboration: As a vital part of our interdisciplinary team, your leadership will enhance our collaborative, supportive environment, ensuring the highest well-being and care standards for our patients.
  • Clinical Supervision and Mentorship: Guide and inspire our caregivers to adhere to care plans with dedication, embodying our values of altruism and integrity, which are crucial to the exceptional care we provide.

What You'll Need to Excel

Licensure

A valid Minnesota Registered Nurse license. Dual licensed in North Dakota is a plus, but not required.

  • Passion for Patient Care: A heartfelt desire to engage directly with patients in their homes, driven by a commitment to positively impact their lives.
