Join our dynamic healthcare team as a Home Care Registered Nurse, where you will play a vital role in delivering compassionate, high-quality nursing care directly to patients in their homes. This energetic position offers the opportunity to empower individuals through personalized health management, promote recovery, and enhance quality of life. As a key member of our home care services, you will coordinate patient care plans, perform initial, routine and 6-month assessments, and collaborate with multidisciplinary teams to ensure optimal health outcomes.

Responsibilities

  • Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop personalized care plans.

  • Collaborate with physicians and interdisciplinary teams on discharge planning, triage decisions, and coordination of services across settings such as assisted living or nursing homes.
  • Reviews and investigates the clinical content, diagnoses, medical history, medications, fee status and other procedures from the records of clients
  • Directs staff toward the appropriate use of community resources
  • Appraises staff performance during their probationary period and annually, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff
  • Identifies and evaluates client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff
  • Develop the Plan of Care after assessing the client and consulting the client and/or family member/responsible person
  • Review and update Care Plans as needed but no less then every 90 days.
  • Participates in community related functions as necessary
  • Assists with the orientation of new professional staff to familiarize them with Agency policies and procedures
  • Secure physician orders at least every three months for continued care unless there is a pre-ordered period of no more than a year (for Medicaid patients)
  • Sees clients in the home setting as needed
  • Participates in clinical record reviews
  • Reviews each schedule for discharges and aide supervisory visits

Qualifications

  • Valid RN license in good standing with current state requirements.
  • Valid Driver's License

Pay: From $100.00 per hour Expected hours: 5.0 – 10.0 per week
