As a Home Health Registered Nurse (RN), you will provide skilled nursing services throughout Maricopa County and surrounding Arizona communities. Responsibilities include:

Conduct comprehensive nursing assessments in patients' homes Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring Develop, implement, and update individualized home health care plans Educate patients and caregivers on disease management, medications, safety, and ongoing care needs Collaborate with physicians, case managers, therapists, and interdisciplinary care teams Complete accurate and timely clinical documentation in compliance with agency and regulatory standards Monitor patient progress and communicate changes in condition to the care team Support improved patient outcomes, independence, and quality of life Qualifications

Current Arizona Registered Nurse (RN) license in good standing Graduate of an accredited nursing program (BSN preferred) Minimum of 1 year of RN experience required Home Health nursing experience strongly preferred Experience working with geriatric patient populations preferred Strong clinical assessment and critical thinking skills Excellent communication and organizational abilities Valid driver's license, reliable transportation, and ability to travel throughout the East Valley, Scottsdale, and North Phoenix areas Ability to work independently while delivering compassionate mobile care Apply Today
