Responsibilities
Conduct comprehensive assessments and develop individualized care plans
Deliver skilled nursing care in accordance with physician orders and organizational standards
Monitor patient status, evaluate responses to treatment, and document all care accurately
Educate patients and families on disease management, medications, and home safety
Collaborate with interdisciplinary team members to ensure continuity of care
Qualifications
Education
- Must have an Associates Degree in Nursing. (Must obtain BSN within 12 years of hire date)
Certification/License
- Active Iowa Registered Nurse licensure issued by the Iowa Board of Nursing
- Within 1 year of hire, need to obtain RN licensure in the State of Wisconsin
- Must have a valid driver's license and proof of automobile insurance coverage
