About this role
Seeking Experienced Home Health Care RN's. Providing home health care services to meet the clinical needs of the individuals within facilities and their homes. Responsibilities include regularly re-evaluating patient nursing needs, initiating plan of care, using health assessment data to determine nursing diagnosis, developing care plans, administering medications and treatments, counseling patients and families, providing health care instructions, identifying discharge planning needs, preparing clinical notes, communicating with physicians, participating in on-call duties, and coordinating care plan with community health-related persons.
Minimum of 2 years of home health care experience. Current RN IL license and CPR certification required. Must be a licensed driver with insured automobile.
