Schedule

Monday through Friday 8 am to 5 pm. on call one weekend a month 8am to 8am. on call 2 weeknights a month 5 pm to 8am. work two holidays a year on a rotation. Territory: Macedonia, Twinsburg, Aurora, Hiram, Hudson, Kent, Streetsboro, Kent, Ravenna, Rootstown, Cuyahoga Falls, Stow areas Guaranteed Base Hourly Rate + Uncapped Bonus Opportunities Mileage Reimbursement Provide Meaningful Care Beyond the Bedside Join Summa Home Health as a Licensed Practical Nurse and make a direct impact by helping patients recover safely and comfortably in their own homes. What You’ll Do

Provide skilled nursing care in patients’ homes Follow physician orders and care plans created by the Case Manager Monitor patient conditions and report changes Administer medications and treatments as prescribed Educate patients and families using teach-back methods Help reduce hospital readmissions through patient education and preventative care Complete clinical documentation in accordance with agency and Medicare guidelines Communicate schedule or availability changes with the appropriate team members Qualifications Current Licensed Practical Nurse (LPN) license Minimum of 1 year of LPN experience in an acute care setting Home health experience preferred Valid driver’s license and active auto insurance Strong communication, time management, and documentation skills Compassionate, dependable, and patient-focused Make a Difference Every Day If you’re an LPN looking for more autonomy, one-on-one patient care, and the opportunity to support patients throughout their recovery journey, we’d love to connect with you.
