Organize and complete patient assignment in a timely manner seeking assistance as needed Delegate appropriately and effectively; make appropriate decisions and use sound judgment based on national nursing standards and Practice Act. Identify and document the patient and family’s physical and emotional responses to care delivered. Accountable to establish, implement, communicate and evaluate the interdisciplinary plan of care with outcomes.

Utilize assessment data gathered from patient and family to ensure completion of data base and plan of care within identified timeframes. Patient/Family Advocacy Identify learning needs and contribute to teaching of patient/family based on plan of care. Seek assistance to meet patient’s educational needs.

Accountable to deliver patient care and service in a manner that is sensitive to diversity. Communication reflects development of plan of care and ability to advocate to meet patients’ needs. Safety Speak up when team members appear to exhibit unsafe behavior or performance Continuously validate and verify information needed for decision making or documentation Stop in the face of uncertainty and takes time to resolve the situation Demonstrate accurate, clear and timely verbal and written communication Actively promote safety for patients, families, visitors and co-workers Attend carefully to important details - practicing Stop, Think, Act and Review in order to self-check behavior and performance
