About this role
Full-time RN Case Manager position coordinating hospice nursing services. Conducts patient assessments, develops care plans, monitors patient progress, provides patient and caregiver education, supervises home health aide staff, and serves as liaison between patients, families, physicians, and interdisciplinary team members. Day shift 8:30 AM - 5:00 PM with every fourth weekend required and holiday rotation.
