Key Responsibilities and Duties

  • Conduct comprehensive initial and ongoing patient assessments, including physical, psychosocial, and environmental evaluations.
  • Develop, implement, and update individualized plans of care based on physician orders, patient needs, and evidence-based nursing practice.
  • Perform skilled nursing services including medication administration, wound care, catheter care, IV therapy, infusion therapy, ostomy care, disease management, and other physician-ordered treatments.
  • Monitor patient condition, identify changes in clinical status, and promptly communicate significant findings to the physician and interdisciplinary care team.
  • Complete medication reconciliation and educate patients and caregivers regarding medications, disease processes, treatment plans, infection prevention, and self-management techniques.
  • Coordinate care with physicians, therapists, medical social workers, home health aides, and other healthcare professionals to ensure continuity of care.
  • Complete all required documentation, including OASIS assessments, care plans, visit notes, recertifications, transfer summaries, and discharge documentation in accordance with agency, state, and federal requirements.
  • Supervise Licensed Practical Nurses (LPNs), Home Health Aides (HHAs), and other assigned personnel to ensure services are provided safely and in accordance with the patient's plan of care.
  • Participate in discharge planning and ensure appropriate transition of care when skilled nursing services are no longer required.
  • Respond appropriately to emergency situations and implement interventions within the scope of nursing practice.
  • Maintain compliance with all applicable federal, state, accreditation, and agency policies regarding patient care, documentation, infection control, and confidentiality.
  • Maintain professional competency through continuing education, annual in-service training, and participation in agency quality improvement initiatives.

Education & Licensing

  • Must be licensed as a Registered Nurse (RN) in the State of Arizona.
  • Graduate of an accredited school of nursing.

Certifications

  • Valid BLS certification (e.g., American Heart Association or American Red Cross).

Experience

  • 1–2 years of home health nursing experience

Pay: $40.00 - $50.00 per hour Work Location: In person
