Hartford HealthCare at Home, the largest provider of homecare services in Connecticut. As part of a person-centered care model, this role provides skilled nursing care to clients in home and community-based settings ensuring the delivery of outcome based, high quality, cost-effective care. Responsibilities include maintaining client records, making follow-up visits to assess disease progress, carrying out the plan of care, teaching patients and reinforcing caregiver education, coordinating care with providers, directing LPNs and Home Care Aides, and referring clients to community resources.

Requirements

RN License (registered nurse with license to practice in CT)
Graduate of an Associate's Degree or diploma program in nursing
One year nursing experience or one year nursing experience in a specialty
In-home healthcare experience preferred
Home health agency experience preferred
BSN preferred (Graduate of NLN approved BSN program)
Medical/surgery or home care experience preferred
