Job Overview

As a Licensed Practical Nurse (LPN) on the Memory Care Unit, you’ll provide attentive nursing care to residents living with dementia and Alzheimer’s disease. You’ll monitor health needs, administer medications, document care, and partner with residents, families, and caregivers to support comfort, dignity, and daily well-being.

Your impact: By recognizing changes in residents’ health and communicating them promptly, you help residents receive timely care and give families and care teams clearer guidance.

Responsibilities

  • Assess residents, monitor vital signs, and report changes in condition to the appropriate clinician.
  • Administer medications and treatments within your scope of practice; support catheter care, tube feeding, and IV infusion when assigned and authorized.
  • Provide memory care, senior care, and patient care with respect for each resident’s preferences and abilities.
  • Maintain accurate medical documentation in electronic health records (EHR) and follow Health Insurance Portability and Accountability Act (HIPAA) privacy requirements, infection control practices, and long-term care regulations.
  • Coordinate with caregivers and the care team on care plans, pain management, intake, and family communication.

Experience

  • Required: Current LPN license and ability to provide nursing care within applicable regulations and scope of practice.
  • Preferred: Experience in memory care, dementia care, Alzheimer’s care, geriatrics, assisted living, long-term care, nursing homes, skilled nursing facilities, or post-acute care.
  • Familiarity with medication administration, patient assessment, vital signs, medical terminology, and electronic medical record (EMR) or EHR systems is an asset.
  • Additional experience in hospice care, acute care, triage, or working with individuals with developmental disabilities is welcome.
  • Pay: $20.00 - $33.00 per hour
  • Expected hours: 25.0 – 35.0 per week
  • Work Location: In person
