Summary
The Licensed Practice Nurse (LPN) is accountable for providing nursing care to assigned patient under the direct supervision of the registered nurse (RN). The LPN is responsible for the accurate and timely administration of medications and treatments and is accountable for delegating and practicing nursing care within legally established guidelines and scope of practice.

Responsibilities
Demonstrates commitment to patient by participation in the implementation of the nursing process in providing patient care under the direction of an RN.
Provides direct patient care within their scope of practice to all patients as delegated by the RN in an efficient and respectful manner, observing and documenting patient responses to nursing care and medications, and adjusting care in collaboration with RN based on patient responses.
Administers ordered medication and documents treatment appropriately and in a timely fashion.
Possesses skills/knowledge to perform treatments (i.e. medications, equipment)
Contributes to the assessment, planning, and evaluation of patient care by collecting data;
providing prompt notification of changes in patient condition to appropriate individual;
documents nursing care delivered and patient responses; implements of plan of care in knowledgeable and skillful manner, as evidence by team feedback and documentation; involves family and or significant other in plan of care.
Performs other duties as assigned or requested. Collects data and provides input to the RN/health care team to assist in developing and updating an individualized plan of care for the patient. Accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family.
Observes, recognizes, and reports information/changes related to the physical, spiritual, and psychosocial needs of the patient and takes action as appropriate.
Collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning.
Reports and documents appropriate information related to the patient/family regarding patient care, teaching/learning, and transitions in care needs.
Verifies that patient charts are updated promptly and accurately.
Delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care plan.
