About this role

The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve.

We are currently looking for qualified candidates to join MNHC as a Registered Nurse - Disease Management. _*Please note: This role requires applicants to be bilingual in English and Spanish. *_This position reports to the Senior Nurse Manager and is Exempt role with a starting hourly range of $47.32 - $50.95 (depending on years of clinical experience post RN license)

The Registered Nurse is responsible for the delivery of quality nursing care and will assist in establishing a clinical team approach to comprehensive medical care and provide continuity of care to MNHC patients at multiple locations.

*Essential Functions/

Responsibilities

*

  • Coordinates patient flow in clinical area.
  • Responsible for obtaining and recording accurate medical information and history. This includes the following:

Checking patient charts

Checking encounter forms, prescriptions, lab and X‑ray requests.

Completing disability, CHPD, and other medically related‑ forms.

Maintaining tracking systems and follow-up reports/requests.

Initiating health maintenance and prevention procedures.

  • Assesses initial health needs of patients and prepares them as indicated or as condition warrants.
  • Provides medical treatment as ordered and performs procedures according to MNHC nursing protocol.
  • Prepares and administers medication as ordered.
  • Responsible for triage/emergency and medical screening of patients in charge area.
  • Provides patient education and counseling.
  • Provides telehealth visits, including hospitalization pre and post discharge planning
  • Attends and participates in nursing staff meetings as needed.
  • Maintains confidentiality of all patient information according to federal guidelines and regulations.

  • Valid California Registered Nurse License.
  • Bilingual in English and Spanish.

  • Two years of experience in ambulatory care clinic or hospital experience.
