About this role
PRN Registered Nurse, Home Health. James River Home Health is actively seeking a compassionate and dedicated PRN Home Health RN Case Manager to support patients in the Prince William territory. In this role, you will manage a caseload of patients in their homes while collaborating with the interdisciplinary team to ensure a seamless, high-quality care experience. Responsibilities include providing skilled nursing care to homebound patients, coordinating and managing multidisciplinary services, educating patients and families on health maintenance and disease prevention, and completing timely electronic documentation in Homecare Homebase.
Requirements
- Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
- At least one year of home health experience preferred
- Graduate of an accredited nursing school
- Must have a valid driver's license and a reliable car available during work hours
- OASIS and HCHB experience preferred
