About the Role

At Touchstone Health in San Antonio, we specialize in what happens after the hospital — the transition home. As our Home Health Clinical Manager, you'll lead the clinical team that makes those transitions safe, effective, and patient-centered. Lead day-to-day clinical operations, including supervision, staffing, and delegation across nursing and therapy disciplines Recruit, orient, and conduct ongoing competency validation for clinical staff in compliance with Medicare Conditions of Participation and state licensure standards Oversee individualized care plans, conduct care conferences, and perform record audits to ensure regulatory and quality compliance Coordinate interdisciplinary team services and maintain strong relationships with physicians, community partners, and internal support teams Monitor quality outcomes through active participation in QAPI, infection prevention, and emergency preparedness Ensure accurate, timely clinical documentation in the EMR across all patient records Investigate and report patient care concerns, safety incidents, and employee well-being issues promptly and thoroughly

Requirements

What We're Looking For

Active, unrestricted RN license in Texas; graduate of an accredited nursing program 3–5 years of clinical experience, with at least 1 year in home health or a closely related setting Proven ability to supervise clinical staff, coordinate care, and navigate regulatory compliance Strong working knowledge of Medicare and Medicaid Conditions of Participation Experience conducting staff orientation, training, and competency validation Clear communicator and collaborative leader — comfortable working with interdisciplinary teams, physicians, and community partners Proficient with EMR systems and standard clinical documentation practices Organized, decisive, and able to manage multiple priorities without losing sight of patient outcomes
