As a Registered Nurse Case Manager (RNCM), you'll make a meaningful impact by delivering one-on-one, home-based care that helps patients spend quality time with loved ones in the comfort of home. You'll work at the top of your license, using clinical judgment and leadership skills to coordinate care, support families, and guide patients through their recovery. You'll collaborate with a compassionate interdisciplinary team focused on excellent, patient-centered outcomes.
We make it easy for you to focus on patient care. Guaranteed base salary + generous uncapped bonus. $10,000 sign-on bonus for full-time roles. Mileage reimbursement and additional bonus opportunities.
