About this role

Matrix Home Care is seeking an experienced Director of Nursing (DON) to oversee the clinical operations of our West Palm Beach home health branch.

Matrix Home Care has been providing home health services since 1999 and operates multiple branches throughout Florida. We are committed to providing high-quality, compassionate care while supporting our clinical teams and maintaining strong regulatory and operational standards.

The DON is responsible for ensuring quality patient care, clinical compliance, staff performance, and day-to-day clinical operations. This position works closely with the Administrator and provides leadership and oversight to nursing and clinical staff.

Direct and coordinate all clinical patient care services.

Oversee Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and skilled nursing visits.

Review OASIS documentation for accuracy, completeness, and timeliness.

Review patient charts and clinical documentation for quality and regulatory compliance.

Ensure compliance with Medicare, Medicaid, Florida AHCA, OASIS, and applicable accreditation requirements.

Maintain continuous survey readiness.

Lead and support the branch Quality Assessment and Performance Improvement (QAPI) program.

Oversee clinical staffing, scheduling, productivity, and on-call coverage.

Recruit, train, supervise, evaluate, and develop clinical staff.

Address clinical concerns, patient care issues, and staff performance matters.

Coordinate with physicians, therapists, referral sources, patients, and families to ensure continuity of care.

Assist the Administrator with overall branch operations and achievement of clinical and organizational goals.

Competitive salary: $83,000–$94,000, depending on experience
