About this role
Per Diem Hospice Registered Nurse position at Apex Hospice providing end-of-life care including hospice admissions, comprehensive patient assessments, individualized plans of care, symptom management, patient and family education, and interdisciplinary care coordination. Flexible per diem scheduling with local territories in Southern California.
Requirements
- Current California Registered Nurse (RN) License
- Minimum one year of Hospice RN experience required
- Current BLS Certification
- Hospice case management experience preferred
- Knowledge of Medicare Hospice Conditions of Participation
- Excellent communication, assessment, and critical thinking skills
- Reliable transportation, valid driver's license, and current auto insurance
- Experience with HospiceMD is a plus
