Job Title: RN Visit Nurse – Home Health, 1:1 Patient Care | $4,000 Sign-On Bonus Company: Home Health Care, Inc. Location: Greater Twin Cities Metro Area Schedule: Full-Time with On-Call and Weekend Rotation Compensation: $75,000 to $100,000 per year plus a $4,000 Sign-On Bonus The Impact You'll Make Every visit gives you the opportunity to improve someone's quality of life. You'll care for patients recovering from illness, surgery, injury, or chronic medical conditions while helping them remain independent in the comfort of their own homes. What You'll Be Doing Provide skilled home health nursing visits throughout your assigned territory Complete comprehensive patient assessments Perform wound care, medication management, injections, and other skilled nursing services Monitor patient progress and communicate changes to physicians Educate patients and caregivers on disease management and treatment plans Coordinate care with therapists, social workers, and interdisciplinary team members Complete timely documentation within our Homecare Homebase (HCHB) EMR Help reduce hospitalizations through proactive, high-quality patient care

Requirements

Active Minnesota Registered Nurse (RN) license
Previous nursing experience in home health, hospital, rehabilitation, skilled nursing, or clinical care
Strong assessment and critical thinking skills
Ability to work independently while collaborating with an interdisciplinary team
Excellent communication and organizational skills
Reliable transportation and willingness to travel throughout the local service area
Homecare Homebase (HCHB) EMR experience
Rotating weekends and on-call availability
