What We're Looking For
Education
4 Year / Bachelors Degree BSN, preferred.
Experience
3 years of experience in Telemetry, CCU, Cardiac Stepdown; Minimum of 3 years of Clinical RN experience, required. Licensure/
Certification
Registered as RN in NC or compact state, required. Certification as a CHFN, required.
Additional Skills Required
Additional Skills/Requirements (required) Demonstrates interdisciplinary leadership skills, superior interpersonal communication skills, and able to communicate patient’s needs to providers. Organization skills, critical thinking skills, and clinical experience/expertise in HF management Ability to work independently on their own as well as collaborative with the interdisciplinary team.
What You'll Do
Collaborate with the Heart Failure team in all aspects of patient care, coordinating care needs of the heart failure population from the time of referral through end-of-life. Provide disease self-management education and serve as a resource for heart failure patients. Actively participate in performance improvement initiatives to enhance care, processes, and outcomes. Triage patient phone calls and collaborate with medical providers to titrate medications within scope of practice.
