About the Role
We are seeking a highly skilled and compassionate Licensed Vocational Nurse to join our Home Health and Hospice team in Fresno, CA. As an On-Call LVN for Home Health and Hospice, you will provide clinical support, patient assessment, and triage for home health and hospice patients during after-hours, weekends, and other designated on-call periods.

Qualifications
- Active and unrestricted LVN license in the state of California
- Minimum of 1 year of recent experience in Home Health or related field preferred
- Current CPR certification
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Responsibilities
Answer all after hours and weekend phone calls. Triage patient needs - If issue cannot be resolved via phone/facetime then a skilled visit will need to be performed for Home Health calls. Most hospice issues/incidents will need a skilled visit. Coverage area will be the Fresno and Visalia Area Territories. Updating on-call chat and patient charts as needed. Communicate with physicians/RNs/care teams.
Skills
The On-Call LVN should possess strong clinical assessment, patient triage, and critical-thinking skills, with the ability to recognize changes in patient condition and respond appropriately. Strong communication and interpersonal skills are essential when working with patients, families, caregivers, and members of the interdisciplinary care team. The ideal candidate will be able to work independently, prioritize competing needs, make sound clinical decisions, and remain calm and responsive in urgent or high-stress situations.
Excellent organization, time management, problem-solving, documentation, and electronic medical record (EMR) skills are also required. A compassionate, patient-centered approach, along with flexibility and dependability for after-hours and weekend on-call coverage, is essential.

This position follows a 7 days on, 7 days off on-call schedule, including after hours and weekends.
