Forest Park Hospice is seeking a compassionate and dependable PRN Registered Nurse Case Manager (RNCM) to serve patients and families throughout the Yamhill County area (McMinnville, Newberg, Sheridan, Yamhill). This position will initially manage a small caseload of approximately five to six hospice patients, allowing the RNCM to provide attentive, individualized care while helping us expand our presence in the community. As the Yamhill County caseload grows, this position has the potential to develop into a full-time opportunity.
