Registered Nurse (RN)/ Case Manager PRN/Per Diem. Seeking a qualified individual to join the Lucent Health, Home Health Services team. Responsibilities include performing admission, transfer, re-certification, resumption of care, and discharge OASIS and visits for home care patients, submitting clinical notes, and discussing patient problems with supervisors. Must have driver's license and be able to drive to patients' residences. Physical activities include walking, sitting, stooping, standing, and lifting patients. Must communicate verbally and in writing by telephone in English.

Requirements
- Florida RN License
- Graduate from an accredited School of Nursing
- Licensed in the state of Florida as a Registered Nurse
- One or more years of experience as Home Health nurse
- Knowledge of Medicare and Medicaid guidelines (preferred)
- Criminal background check
- Current CPR certification (online certification not accepted)
- Driver's License
- BLS Certification
