Registered Nurse (RN)/ Case Manager PRN/Per Diem. Seeking a qualified individual to join the Lucent Health, Home Health Services team. Responsibilities include performing admission, transfer, re-certification, resumption of care, and discharge OASIS and visits for home care patients, submitting clinical notes, and discussing patient problems with supervisors. Must have driver's license and be able to drive to patients' residences. Physical activities include walking, sitting, stooping, standing, and lifting patients.
