Care Coordinator

This is a great opportunity for an experienced RN who enjoys building relationships with residents and families, supporting and developing staff, and having a direct impact on quality of care.

• Monday–Friday, 8:00 AM–4:30 PM
• Occasional on-call needs

*What You'll Do*

• Lead, coach, and support the nursing team within your assigned neighborhood
• Oversee resident care and care planning to ensure high-quality, person-centered care
• Build strong relationships with residents and families
• Identify concerns early and partner with the team to develop solutions
• Mentor and develop nursing staff
• Collaborate with Social Services, Activities, and other departments
• Support clinical quality, regulatory compliance, and continuous improvement
• Serve as a resource and leader for residents, families, and staff

*What We're Looking For*

• Current Wisconsin RN license
• Long-term care or skilled nursing experience strongly preferred
• Previous leadership, charge nurse, preceptor, or supervisory experience preferred
• Strong communication, organization, and problem-solving skills
• A supportive leadership style and desire to develop others
