Responsibilities
- Collect data on residents’ physical and psychosocial status and communicate findings to the supervising RN and the interdisciplinary team.
- Assist in developing an individualized plan of care reflecting nursing diagnoses and interventions, involving residents and families to achieve desired outcomes.
- Recommend revisions to the plan of care after evaluating resident outcomes and coordinate referrals as needed.
- Collaborate with residents, families, and interdisciplinary colleagues to promote a safe, respectful, and culturally sensitive care environment.
- Maintain current nursing knowledge and competency, participate in orientation/mentoring of staff, and provide leadership under the direction of the RN.
- Adhere to safety and infection control practices; apply knowledge of growth and development for adult and geriatric patients; respond to potential exposure to blood or body fluids.
Requirements
- Active New York LPN licensure
- BLS certification (American Heart Association) or RQI
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to provide care suitable for adult and geriatric residents
- Strong communication skills and ability to work under RN supervision in a team-based environment
