About this role
Responsibilities
- Make rounds in Skilled Nursing Facilities to provide primary care to residents.
- Provide comprehensive primary care services to geriatrics patients, including dementia care.
- Perform physical examinations, diagnose and treat common acute illnesses and injuries.
- Develop and implement treatment plans for chronic conditions and manage ongoing care.
- Order and interpret diagnostic tests, such as medical imaging and laboratory tests.
- Prescribe medications, including psychopharmacology, and provide patient education on medication use.
- Collaborate with other healthcare professionals to coordinate patient care, including discharge planning.
- Maintain accurate and up-to-date medical records using eClinicalWorks or similar electronic health record system.
- Communicate with office staff to address faxes and telephone call messages from facilties.
Experience
- Must have a valid Nurse Practitioner license in the state of Illinois.
- Strong knowledge and experience in geriatrics, dementia care, and other primary care services preferred.
- Proficiency in using eClinicalWorks or similar electronic health record system.
- Excellent communication skills to effectively interact with patients, families, and healthcare team members.
- Ability to work independently and collaboratively in a fast-paced environment.
