  • Observe and monitor patients’ health status, including vital signs and overall condition, to ensure safety and well-being.
  • Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support.
  • Administer medications accurately following medical documentation protocols while adhering to HIPAA regulations for patient privacy.
  • Support clients with specialized care needs including Alzheimer’s care, dementia care, hospice support, catheter care, and memory care.
  • Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities precisely and maintain medical records.
  • Utilize assistive devices like Hoyer lifts safely during patient transfers to prevent injury.
  • Prepare nutritious meals and assist with light cleaning, laundry, and household chores to promote a clean and healthy environment.
  • Collaborate with healthcare professionals by documenting medical information accurately and communicating any changes in patient condition promptly.
  • Provide compassionate companionship and emotional support to foster a positive home environment for clients.
  • Follow all safety protocols including CPR certification requirements to ensure preparedness for emergencies.

*Qualifications*

  • Prior experience in home health, assisted living, nursing homes, or long-term care settings is highly desirable.
  • Knowledge of medical terminology, medication administration, catheter care, hospice care, dementia care, and Alzheimer’s support is preferred.
  • Demonstrated ability to work with individuals with developmental disabilities or other special needs.
  • Strong patient observation skills combined with the ability to assist with ADLs effectively.
  • Familiarity with EMR/EHR systems for accurate documentation of medical records and care activities.
  • Valid CPR certification; additional certifications such as HIPAA training are advantageous.
  • Experience working with patients requiring memory or dementia care is a plus.
  • Ability to handle physical tasks including lifting using Hoyer lifts and providing mobility assistance safely.
  • Excellent communication skills coupled with a caring attitude to build trust and rapport with clients.
