Hospice Director of nursing

Qualifications

• Current and active California Registered Nurse (RN) license
• Must meet all applicable California requirements to serve as a hospice Director of Patient Care Services (DPCS)
• Must meet one of the following:
• Bachelor’s degree or higher in nursing or another health-related field and at least 2 years of full-time supervisory or managerial experience in a hospice or home health agency within the last 5 years, OR
• At least 4 years of full-time supervisory or managerial experience in a hospice or home health agency within the last 5 years
• Previous hospice DON/DPCS or nursing leadership experience
• Hands-on hospice patient care and field nursing experience
• Experience with hospice clinical operations and in-office responsibilities
• Strong understanding of Medicare/CMS hospice requirements and California hospice regulations
• Knowledge of HIPAA requirements and ability to maintain patient privacy and confidentiality
• Detail-oriented, organized, reliable, and punctual
• Strong clinical documentation and chart review skills
• Must be able to work both in the office and in the field

Schedule & Availability

• Part-time office hours with flexible days
• Must be available to provide field nursing support as needed
• Must have 24-hour on-call availability for urgent field nursing needs as required

Key

Responsibilities

• Oversee and supervise hospice clinical and patient care services
• Ensure high-quality, compassionate, and compliant hospice care
• Supervise and support nursing and clinical staff
• Conduct patient visits and provide direct patient care when needed
• Review patient charts and clinical documentation for accuracy, completeness, and timely completion
• Ensure assessments, plans of care, visit notes, and other required documentation are completed appropriately
• Participate in and oversee interdisciplinary group (IDG) care coordination
• Ensure clinical services comply with Medicare/CMS requirements, California hospice regulations, HIPAA, and agency policies and procedures
• Maintain the confidentiality and security of protected health information (PHI)
• Assist with QAPI activities, audits, surveys, corrective actions, and ongoing compliance
• Provide clinical leadership and support to the hospice team
• Assist with on-call and urgent patient needs as required

The ideal candidate is comfortable managing the clinical side of the agency while also stepping into the field to support patients and the nursing team when needed.
