Registered Nurse

• *Patient assessment:* Conduct comprehensive initial and ongoing assessments of patients' physical, psychosocial, and environmental needs, monitoring and documenting vital signs, symptoms, and overall health status.
• *Care planning:* Develop, implement, and update individualized, goal-oriented care plans in collaboration with the patient, family, and physician.
• *Medication and treatment administration:* Administer and monitor the effectiveness of medications, injections, IV therapy, and other treatments as prescribed by the physician.
• *Wound and catheter care:* Provide skilled wound care, including dressing changes and assessment of healing progress. Manage ostomy and catheter care as needed.

education

• Instruct patients and caregivers on medication management, disease processes, wound care, pain management, and other home care procedures.
• *Coordination of care:* Act as a case manager by coordinating with the patient's physician, physical therapists, occupational therapists, social workers, and home health aides to ensure seamless, holistic care.
• *Documentation:* Maintain accurate, timely, and complete clinical records using electronic health records (EHR) in compliance with agency policies and regulatory standards.
• *Communication:* Communicate effectively with patients, family members, and the healthcare team to report changes in a patient’s condition and ensure continuity of care.
• *Advocacy and support:* Serve as a patient advocate, providing emotional support and guidance to patients and families during challenging times.
• *Emergency response:* Respond to and manage emergency situations that may arise during a home visit.

Qualifications

Licensure

• Current, active Registered Nurse (RN) license in the state of practice.
• *

Education

• Associate Degree in Nursing (ADN) is required; Bachelor of Science in Nursing (BSN) is preferred.
• *

Experience

• A minimum of 1–2 years of recent clinical experience, with home health experience strongly preferred. Other relevant experience may include medical-surgical, intensive care, or geriatric nursing.
• *

Certifications

• Current CPR certification is required.
• *Transportation:* Reliable transportation, a valid driver's license, and proof of current auto insurance are necessary for travel to patient homes.
