Position Summary for Visiting Registered Nurse (RN)

The RN Visiting Nurse is responsible for answering all calls, meeting patient needs, and making visits. Responsible for providing support to patients, and family members following the plan of care. They will be responsible for answering calls for clinical issues.

  • Reviews clinical referral information and responds to requests and inquiries as appropriate. Collaborates with the Director of Patient Care regarding the eligibility and appropriateness of clients for hospice services.
  • Will do an ongoing comprehensive assessment of the patient and family to determine hospice needs.
  • Provides a complete physical assessment and history of current and previous illness(es) while being aware of the physical, emotional, and spiritual aspects. Gather data on social, economic, and cultural factors which may influence health, well-being, and quality of life.
  • Assumes responsibility to coordinate patient care for assigned caseload and provides direct care to patients maintaining the highest level of comfort and quality of life.
  • Applies nursing principles and techniques in the treatment of patients.
  • Provides nursing care, emotional support, and education to patients and families.
  • Encourages patients and family members to participate in the patient's plan of care to meet established goals.
  • Coordinate with hospice physicians with required hospice MD in person or virtual face-to-face.
  • Participates in On-Call duties as assigned and defined by the Director of Patient Care.
  • Supervise LVNs and HHAs - ensuring they are following the patient's plan of care.
  • Participates in the on-call and holiday work rotation.
  • Will be trained on how to do hospice admissions as a backup.

Qualifications
