Position
Summary
Lakes Region VNA and Hospice is seeking a dedicated and compassionate Registered Nurse (RN) to join our team. This role is crucial in supporting the after-hour needs of our Hospice and Homecare patients. The RN will work Sunday through Thursday, 4pm – 8pm and Monday through Thursday, 5am – 8am. The primary responsibilities include triaging patient calls, doing pronouncements as well as home care and hospice visits to patients in their homes as needed.

Responsibilities
- Triaging patient calls escalated from the on-call triage service and providing appropriate guidance and support.
- Making visits to patients’ homes to assess and address their needs.
- Collaborating with the interdisciplinary team to ensure comprehensive patient care.
- Documenting patient interactions and care to ensure they are provided accurately and timely.
- Providing emotional support and education to patients and their families.
- Provide end of life support for patients and families.
- Adhering to infection control protocols and safety guidelines.
- Participating in team meetings and ongoing training sessions.
Qualifications
Education
Bachelor’s degree in nursing (BSN) preferred.
Certification
Current and valid RN license in NH.
Experience
Previous experience in a home health care and hospice setting is preferred.
Skills
o Strong communication and interpersonal skills.
o Ability to work effectively with staff.
o Excellent organizational and time management skills.
o Knowledge of state and federal home health care regulations.
o Ability to work independently and as part of a team.
o Must be self-motivated, dedicated, detail oriented.
o Compassionate, patient, and empathetic demeanor.
o Reliable transportation and a valid driver’s license.
