Licensed Practical Nurse (LPN) - Home Health

We are seeking Licensed Practical Nurses (LPNs) who are passionate about providing exceptional care in the Ankeny, IA area. As a Home Health LPN, you’ll provide one-on-one skilled nursing care in the home, following a patient-specific care plan developed by the RN Case Manager. You’ll work collaboratively with a supportive team of healthcare professionals to ensure the best outcomes for our patients. What You’ll Do Provide skilled nursing care in the home as outlined in the plan of treatment. Assess patient conditions, communicate changes, and document accurately. Perform wound care, disease management, and medication reviews. Educate patients and families on care plans and safe practices. Collaborate with physicians, RNs, and the interdisciplinary team. Qualifications Current, valid Iowa LPN license Minimum 1 year of home health experience. Valid driver’s license, reliable transportation, and auto insurance. Current Iowa Dependent Adult Abuse Mandatory Reporter Training or ability to obtain upon hire. Current CPR certification.
