About this role
The Clinical Nurse RN is responsible for ensuring quality total patient care through case management, assessment, interventions, and evaluation of patient needs through nursing care plans for each assigned patient. Performs initial and ongoing comprehensive assessments of the client's needs at appropriate time points for home health care or hospice patients. Acts as a case manager, coordinating all patient/family services and prioritizing the patient needs with the members of the interdisciplinary team. Supervises LPN/s and paraprofessionals providing home health care or hospice services.

Requirements
- Must have a Compact RN License and/or Registered Nurse licensed in the state(s) of operation, required Must possess sound knowledge of nursing principles, procedures, and standards of practice as determined by state and federal regulations, required A minimum of one year of licensed professional nursing experience is preferred; Experience in community or home health preferred
