About the Role

Touchstone Health is seeking an experienced, compassionate Registered Nurse to join our hospice team in San Antonio, TX. This is a highly autonomous position with a 7-on / 7-off schedule (Friday to Friday, 5:00 PM to 8:00 AM & every other weekend days/nights), offering a unique rhythm of deep focus and meaningful time off.

Responsibilities

  • Deliver nursing services according to the established plan of care.

  • Conduct initial evaluation visits and regularly reassess the patient’s nursing needs.
  • Initiate the plan of care and make necessary revisions based on patient condition.
  • Provide specialized nursing services that require substantial nursing skills.
  • Implement preventive and rehabilitative nursing procedures as appropriate.
  • Prepare clinical and progress notes for each patient visit and complete summaries for care conferences per agency policy.
  • Coordinate services and communicate with personnel about changes in patient condition and needs.
  • Counsel patients and their families in meeting nursing and related healthcare needs.
  • Participate in and present in-service programs as needed.
  • Adhere to agency policies and procedures.
  • Process orders, notify the physician of patient needs, and handle changes in patient condition, including completing certifications/recertifications and discharge summaries.
  • Determine the necessary amount and type of nursing for each individual patient.
  • Refer patients to specialized services (e.g., Physical Therapy, Speech Language Pathology) as needed.
  • Supervise and provide teaching to other nursing personnel.
  • Conduct patient care conferences on patients under their care.
  • Engage in peer review and participate in Quality Assurance and Performance Improvement (QAPI) activities as assigned.
  • Provide total patient care as needed.
  • Take on-call duties for nights, weekends, and holidays as assigned.
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant condition changes within agency-defined timeframes.
  • Utilize ICD-10 codes appropriately for accurate documentation.

Requirements

Qualifications

  • Graduate of an approved professional nursing school, with a current RN license in the state of practice.
  • Two (2) years of nursing experience preferred.
  • Acceptance of the agency’s philosophy and goals.
  • Ability to exercise initiative and independent judgment in patient care.

Working environment

Primarily works indoors in agency offices and patient homes, with required travel to and from patient locations.

Reports to: Director of Clinical Services or Clinical Manager Supervises: Licensed Practical Nurse and Home Health Aide

Risk exposure

High risk due to direct patient care in home health settings.

Lifting requirements

The role requires the ability to engage in physical activities essential for patient care, including extended periods of standing, heavy lifting, frequent bending, and other physical activities on a regular basis.
