Job Overview
Country Villa Wellness & Rehabilitation is seeking an experienced Treatment Nurse / Wound Care Nurse (LPN) to join our Skilled Nursing Facility team. This full-time position focuses on wound care management, skin integrity, pressure ulcer prevention, and discharge planning in a long-term care setting.
This role is ideal for an LPN with strong clinical skills and experience in wound treatments, documentation, and regulatory compliance.

Responsibilities
Perform comprehensive skin and wound assessments
Administer wound care and treatment orders as prescribed
Monitor wound healing progress and recommend treatment changes
Document treatments accurately in accordance with LTC regulations
Track and report pressure ulcers and skin alterations
Complete Braden risk assessments and implement prevention plans
Coordinate wound care supplies and inventory
Educate nursing staff on wound care best practices
Assist with discharge planning
Collaborate with physicians, DON, and interdisciplinary team
Participate in QAPI and regulatory compliance activities
Maintain knowledge of current wound care standards and regulations
Qualifications
Licensed Practical Nurse (LPN) – active Missouri license required
Graduate of an approved LPN/LVN program
Minimum 1 year wound care or treatment nurse experience in long-term care
2+ years Skilled Nursing Facility experience preferred
Current BLS/CPR certification
