LPN/LVN – Home Health

We are seeking an experienced *LPN/LVN – Home Health* for a 13-week assignment in Redding, CA. The ideal candidate must have hands-on home health experience, including *HomeCare HomeBase (HCHB) EHR*, wound care, wound vacs, and ostomy care.

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Perform patient assessments and monitor changes in condition.
• Provide wound care and manage wound vacs as needed.
• Assist with ostomy care and patient education.
• Administer medications and treatments according to the plan of care.
• Document patient visits and care accurately in *HomeCare HomeBase (HCHB)*.
• Communicate patient updates with physicians and the healthcare team.
• Educate patients and families on medications, treatments, and disease management.
• Follow infection-control, safety, and home health protocols.

Required Certifications

• *BLS Certification – AHA Required*
• *Valid Driver’s License – Required*
• *Valid Auto Insurance – Required*

Must-Have Experience

• *HomeCare HomeBase (HCHB) experience – Required*
• Previous *Home Health LPN/LVN experience – Required*
• Experience with *wound vacs – Required*
• Experience with *ostomy care – Required*
• Strong general wound-care experience.
• Ability to work independently in a home health setting.
• Excellent communication and documentation skills.

*Monday–Friday | 8-Hour Day Shifts*
