About this role
Find your purpose with CentraCare by joining our Hospice team as an RN Case Manager! This position will serve patients primarily in St. Cloud, Foley, Princeton and surrounding areas. You will make a difference in the lives of others by caring for patients and their families in the comfort of wherever they call home.
Responsibilities include completing medication reviews, seeking orders from physicians, and teaching caregivers about how to provide end of life care for their loved ones. Other routine nursing tasks include wound care, catheter care, head to toe assessments, and pain assessments while tending to ongoing case management with the team. We provide an extensive orientation program as well as ongoing RN office support to you while visiting patients.
Schedule
- Full-time | 72-80 hours every two weeks

Every 4th weekend | Two rotating holidays annually | Rare on-call shifts
Pay and Benefits
- Pay based on experience and degree
- Associate/Diploma in nursing starting at $39.87 per hour
- Bachelor’s in nursing starting at $41.25 per hour
- Master’s in nursing starting at $42.62 per hour
- Pay range: $39.87 - $66.52 per hour
- Milage reimbursement and extra pay for working weekend shifts
- $3,500 hiring bonus for eligible candidates

Qualifications
- Graduate from an accredited school of nursing
- Current Minnesota Registered Nurse license
- Current American Heart Association (AHA) BLS Healthcare Provider card
- Minimum of two years of experience in an acute care setting, at least one year of RN experience
Valid Minnesota driver’s license, reliable transportation, a driving record in good standing, and insurance liability coverage meeting CentraCare’s policy
