Case Manager RN PRN

The Case Manager is responsible for assessing patient needs and developing care plans, providing care to patients in their home, supervising ancillary staff such as LVNs and PT/OT, and providing education to patients and their families. This role has the opportunity to quickly grow into a larger supervisory role for the right candidate. Responsibilities include providing bedside patient care with in-home care experience such as infusions and wound care, supervising clinical staff (RNs, LPNs, therapists, aides), approving and overseeing patient care plans, understanding and complying with Medicare CoPs for visit scheduling, and reviewing charts to ensure quality and regulatory compliance.

This is a PRN role. Pay is based on type of visit: Up to $120 for initial assessment visit (depending on experience) and at least $50 per regular visit.
