Make a lasting impact by helping patients avoid unnecessary hospitalizations while building long-term, meaningful relationships. *Physician Services Group (PSG)* is seeking a *Full-Time Nurse Practitioner (NP)* to join our *Post-Acute Care* team in *Sumter, South Carolina *area. This is a Monday–Friday role offering consistency, clinical autonomy, and a supportive team-based environment. *Schedule & Setting* * Full-Time | Monday–Friday | 8:00 AM–4:00 PM * In-person care at a local Post-Acute & Skilled Nursing Facility * Collaborative care alongside attending physicians and on-site teams *Compensation & Benefits* * Salary: $100,000–$120,000 annually (based on experience) * Annual performance-based bonus * $1,500 CME stipend + paid holidays * Comprehensive benefits: Health, Dental, Vision, 401(k), PTO *Qualifications* * Active Nurse Practitioner license in South Carolina (or ability to obtain) * Board Certified (ANCC, AANP, or equivalent) * Strong clinical judgment and documentation skills * Experience in post-acute, hospital, or outpatient care preferred * Passion for geriatric medicine, preventative care, and chronic disease management *Interested?* Email *Brandon Lewis at BLewis@psgcare.com* to apply or refer a colleague.
