QUALIFICATIONS: Registered Nurse with current license in the state of Wisconsin/Illinois; One year of experience as an RN in an acute care setting, preferably two years of experience. Home Care experience is desired. ESSENTIAL FUNCTIONS: (Other duties as assigned): * Performs assessments and develops individualized patient care plans and manages multi disciplinary care as applicable. * Provides skilled nursing care in a patient's home as ordered by the attending physician in conjunction with the plan of care. * Supervises LPN's and Home Health Aide's in a patient's home. * Complies with agency policies and procedures. Promotes and maintains an agency environment that is in compliance with federal, state, and local regulations. * Submits clear and appropriate documentation in a timely manner and documents patient education. * Communicates effectively to physicians, supervisors, and coworkers in a timely manner. * Collaborating with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients.
