*Weekend On-Call Registered Nurse (RN) - Hospice Job Overview* We are seeking a compassionate *Weekend On-Call Registered Nurse (RN) - Hospice* to provide hospice nursing care to patients in their homes. The RN will coordinate care with the interdisciplinary team, patients, families, physicians, and community partners to ensure comfort, dignity, and quality of life at the end of life. This position requires strong clinical judgment, excellent communication skills, and a passion for hospice and palliative care.
*Weekend On-Call Registered Nurse (RN) - Hospice Responsibilities*
- Perform initial and ongoing comprehensive assessments of hospice patients
- Determine hospice eligibility and appropriate levels of care per Medicare and agency guidelines
- Develop, implement, and revise individualized plans of care
- Educate patients and families on disease progression, symptom management, and end‑of‑life care
- Coordinate care with physicians and the interdisciplinary group (IDG)
- Supervise and provide clinical direction to Hospice Aides and LPN/LVNs
- Monitor caregiver capability and provide education and support
- Complete accurate, timely clinical documentation and progress notes
- Communicate changes in patient condition to physicians and care team
- Follow infection control and standard precautions
- Participate in on‑call rotation as assigned
- Promote agency policies, philosophy, and compliance with all regulations
*Weekend On-Call Registered Nurse (RN) - Hospice Qualifications*
- Graduate of an accredited School of Nursing (Diploma, ADN, or BSN)
- Current, unrestricted Texas RN license
- Current CPR certification
- Valid Texas driver’s license
- Reliable transportation and current auto liability insurance
- 2+ years of clinical nursing experience required
- Hospice, home health, or palliative care experience required
- Experience with end‑of‑life symptom management required
