About this role

*Gilead Hospice *is expanding our mission and looking for dedicated RN Case Managers to join our high-impact team in Austin.

This role involves working closely with patients on a one-on-one basis while receiving support from a patient care team.

What You’ll Do

*

  • Deliver compassionate, comprehensive hospice care through individualized patient assessments and holistic care plans.
  • Serve as a vital educator and guide for families navigating end-of-life options.
  • Collaborate with a team of healthcare professionals to ensure every patient receives dignity and clinical excellence.
  • Maintain meticulous records and uphold the highest HIPAA standards.

*What We’re Looking For:*

  • Valid Texas RN License and CPR certification.
  • Reliable transportation, a valid driver’s license, and auto insurance are required for community-based care.
  • Previous hospice or home health experience is preferred, but a positive attitude and a drive to improve quality of life are essential.
  • We value patience, motivation, and a deep-seated desire to make a difference.
