About this role

RN Admissions Nurse – Home Health. Complete comprehensive Start of Care (SOC) assessments in patients' homes. Accurately complete OASIS documentation and establish individualized plans of care. Educate patients and caregivers about diagnoses, medications, and home health services. Coordinate care with physicians, therapists, case managers, and other members of the care team. Ensure smooth transitions from hospital, rehabilitation, or skilled nursing facilities to home. Maintain timely and accurate documentation.

Compensation

$150–$200 per completed visit + Mileage Reimbursement.
